Basketball Sign UpStep 1 of 425%Church Basketball Traditions & Expectations Since 2002 Rolling Hills United Methodist Church has sponsored community pickup basketball in our multi-purpose gym. Youth and adults from the church and the community are invited to play in an environment that encourages respect, fair play and recreational competition. The privilege of playing in the gym comes with a number of expectations described below. Respect for Each Other No physical or verbal altercations – verbal or physical threats or assaults will result in expulsion from the gym. Expulsion will be for a minimum of three months, with return by approval of the pastor. No cursing at others. Respect for the Church This is a church facility with an associated preschool. Our behavior and language should be appropriate around children and other church members.Loud profanity, or the abuse of the gym or equipment, is grounds for expulsion.Taking food from the kitchen is grounds for expulsion. No playing under the influence of alcohol or drugs. No smoking on the church grounds. No kicking balls in the gym.No hanging on the rim.Respect for the Opportunity to Play Regular players are allowed to bring an occasional friend. This is not an open gym, and no one should bring more than one friend at a time. Too many players means not enough playing time for everyone. If you are the last person in the gym make sure the lights are off and the doors are locked. The church welcomes players of all age and ability levels. If you feel you are too good to play at this level there are other courts where you can demonstrate your skills. Set up or take down tables or other help as requested by church staff. Risk and Liability Use of the facility is at your own risk. The Church assumes no responsibility for lost or stolen items. The Church assumes no responsibility for any injury that may occur. Players younger than high school age (9th grade) must be accompanied by a parent, who will likely be matched up against them. Children younger than 12 are not eligible for RHUMC pickup basketball. Playing basketball involves certain risks, including the risk of injury. By using the church gym and signing this form, you are:acknowledging those risksvoluntarily assuming such risks andreleasing the church from any liability to the fullest extent possible under California law.Email(Required) I have read and understood the expectations on this page and agree to abide by them.(Required) YesName First Last Street Address Address Line 2 City AlabamaAlaskaAmerican SamoaArizonaArkansasCaliforniaColoradoConnecticutDelawareDistrict of ColumbiaFloridaGeorgiaGuamHawaiiIdahoIllinoisIndianaIowaKansasKentuckyLouisianaMaineMarylandMassachusettsMichiganMinnesotaMississippiMissouriMontanaNebraskaNevadaNew HampshireNew JerseyNew MexicoNew YorkNorth CarolinaNorth DakotaNorthern Mariana IslandsOhioOklahomaOregonPennsylvaniaPuerto RicoRhode IslandSouth CarolinaSouth DakotaTennesseeTexasUtahU.S. Virgin IslandsVermontVirginiaWashingtonWest VirginiaWisconsinWyomingArmed Forces AmericasArmed Forces EuropeArmed Forces Pacific State ZIP Code Phone(Required)Emergency Contact Name First Last Emergency Contact Relationship(Required)Emergency Contact Phone Number(Required)Parental Permission As the parent or guardian of a player under the age of 18, I give permission for my child to play pickup basketball at Rolling Hills United Methodist Church.
